Select Benefit Medical: Covers: Hospitalization, Surgery, Anesthesia, Doctor’s Office Visits, Fully insured Prescription card, Diagnostic X-Ray & Lab, Preventive Care, Life Insurance and Critical Care Benefit Included in Plan. www.multiplan.com
Dental / Vision: Dental care includes Preventive Care (100%), Restorative Care (80%) and Major Dental Procedures (50%). Vision includes Eye Exams (100%), Lenses (100%), Frames and Contact Lenses. www.ameritasgroup.com
Disability: Short Term Disability and Long Term Disability for illness or off-the-job accidents
Term Life: Guaranteed Issue up to $100,000 for drivers & other amounts for Spouse and Children.
Accidental Death & Dismemberment: Guaranteed Issue for members up to $250,000.
Critical Illness: Covers: Heart Attack, Stroke, Major Organ Transplant, Kidney Failure and Specified Cancers.
Cancer Select: Variety of plans available for members and their families.
